Key Tasks for Peer Reviewers
- Training: Attend live training or complete the training above (by December 20, 2024)
- Facility Coordination Meeting: Attend a meeting with Peer Reviewers and Site Contacts from your paired facility (in January 2025)
- Chart Review: Travel to your peer facility and review 3 charts (form due April 11, 2025)
- The charts selected should meet the following criteria:
- All charts (or as many as possible) should include at least 30 days of partial hospitalization treatment
- At least 1 chart per Peer Reviewer should be for an individual who was discharged by completing goals and referred to a lower level of care (including if they went to IOP/OP at any provider)
- At least 1 chart per facility should be for a pregnant individual (if possible)
- The charts selected should meet the following criteria:
- Peer Interview: Complete an interview with your peer at your paired facility (form due April 11, 2025)
- Training Log: Document the time spent on all tasks (form due April 11, 2025)
Submit all completed forms to the Penn State team at PeerReview@psu.edu.
